#1 Stretchers stored in the corridor. These stretchers are obstructing emergency evacuation of patients during a fire

#2 Fire extinguisher being obstructed by supplies. Emergency equipment must be kept free and clear for immediate use.

#3 Medical gas shutoff valves blocked. Oxygen emergency shutoff valves must be immediately accessible. Should there be an O2 fed fire, extinguishment can only be accomplished by shutting off the supply of O2

#4 Unattended Ric with chairs not being used, obstructing corridor. Corridors must be kept clear for unimpeded patient emergency evacuation

#5 Very cluttered corridor totally impeding emergency egress for patients in a fire situation

#6 Fire and smoke doors which are on hold open devices must be kept clear. During a fire, these doors automatically close, segregating the fire compartment.

#7 Combustible storage in an egress corridor creating heavy fire load. Storage is allowed only in designated rooms which are specially built for that purpose.

#8 Unsecured O2 cylinder. Med gases must always be stored in racks or in approved cylinder holders

#9 Smoke/fire door blocked. Fire and smoke doors which are on hold open devices must be kept clear. During a fire, these doors automatically close, segregating the fire compartment.

#10 Blocked sprinkler head. Storage must be kept greater than 18” from the ceiling so as to not block the sprinkler system.

#11 Emergency equipment blocked. Fire alarm pull station, evacuation map/instructions and fire extinguisher can never be blocked and must always be available for immediate use.

#12 Storage of greater than 12 oxygen cylinders. There can be no more than 12 cylinders per smoke compartment.

#13 Emergency exit (stairwell) blocked by linen cart.

#14 Store room fire door blocked open. Doors cannot be blocked/chocked open. Fire rated room doors must always be kept closed and latched to contain any fire.

#17  Fire door latch taped open. This room will not be able to latch, which in a fire, will allow fire and smoke to extend throughout the building.
